Registered Nurse - Caregiver Support Program - Lafayette CBOC

The incumbent provides clinical, evidence-based services/interventions and education for the caregiver and veteran. Incumbent educates staff on caregiver issues; develops resources; and monitors continuous quality improvement activities. The incumbent is a skilled communicator, provides outreach and is capable of developing effective partnerships with local, state, federal agencies as well as non-governmental organizations. The CSP RN is administratively responsible for the clinical programming and daily operations, as assigned, and the management of data related to the program at the facility level. Incumbent possesses advanced knowledge of the services and benefits available for caregivers from VA, as well as other federal, state and private sector entities. The CSP RN is responsible for monitoring the well-being of eligible veterans receiving personal care services as established under sections 1720G (a) (9) of Public Law 111-163. This role requires quarterly monitoring which includes home visits and other methods of monitoring as determined appropriate such as video on demand, tele-health or clinic appointments for caregiver/veteran dyads who require a review of skills and abilities of the Caregiver and well-being of the Veteran. The CSP RN completes in-home initial assessments as determined appropriate for referrals of General Caregivers identified by PACT teams or the Caregiver Support Line. The incumbent is responsible for:
Assessment of veteran's in-home care needs. Assessment of the non-licensed caregiver to determine if adequate care is rendered to meet the veteran's care needs and to fulfill the qualifications for the Program of Comprehensive Assistance Family Caregiver guidelines. Assess the home for safety, identifying any needed structural modifications or additional equipment to successfully care for veteran in the home setting. Coordinate with the Veteran's PACT or other specialty team (i.e., SCI or Polytrauma) to arrange in-home services as needed. Follow mandated protocol for ensuring the veteran's and caregiver's safety and reporting suspected abuse/neglect/exploitation to the appropriate agency(ies) (i.e., Adult or Elderly Protective Services). The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required up to 50 pounds. Use of ceiling lifts /mobile lifts is required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. In addition, the incumbent must be emotionally stable and adjust to different types of personalities, demonstrating cheerfulness, compassion, courtesy and concern. He/she must be quick and dependable in taking emergency action as directed and use initiative in preventing accidents.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
