Louisiana Office of Public Health-STD/HIV Program Louisiana Office of Public Health-STD/HIV Program Lafayette, LA Lafayette, LA Full-time Full-time $45,000 - $50,000 a year $45,000 - $50,000 a year 2 days ago 2 days ago 2 days ago Linkage to Care Coordinator (1) Acadiana CARES in conjunction with the Louisiana Office of Public Health-STD/HIV/Hepatitis Program is currently recruiting for an individual in the Lafayette region to connect newly diagnosed persons into medical care and community-based supportive services, 2) provide treatment adherence services to PLWH who have had two viral load test results greater than 100,000 copies/ml within the last twelve months to educate them on the importance of HIV care and medications in viral suppression, and 3) provide specific medical information and navigation services to individuals who have fallen out of care to re-engage them with routine HIV medical care.
4) connect HIV/HCV co-infected clients to HCV medical treatment.
Excellent skills in community engagement, facilitation, quality assurance and policy and procedure development.
General knowledge of HIV, HCV, and/or STIs, case management, transmission, treatment adherence, health education and risk reduction.
Required:
Qualified candidates must have a Bachelors in a social science or human related field.
Two (2) years of social service experience working with vulnerable populations.
Preferred:
Masters preferred with two (2) years of social service experience working with vulnerable populations.
