Essential Functions:
Assists Quality team in accomplishing annual goals of ACO, HEDIS, and STAR measures, as well as Annual Wellness Visits and Medicaid programs Performs required Quality measurements, inspections, and audits:
Tracks and updates information to ensure performance metrics are met.
Review and close care gaps in a timely manner:
Collects data from practice charts or electronic medical record system to close quality gaps.
Utilize effective population health management tools to increase quality outcomes and decrease healthcare costs across all value-based contracts for Medicare, Medicaid, and Commercial payers Develop and implement effective care management protocols and workflows consistent with evidence based guidelines, practice resources, and systematic workflows to enhance patient care Engage, educate, and assist colleagues, physicians and staff in all Clinically Integrated Networks, MSSP, and value based care contracts Reviews HEDIS/STAR/RISK reports and Care Gaps at least monthly with the VP of Quality in order to coordinate an action plan for improved measure performance and patient outcomes Actively participates in team meetings; provides input on practice performance.
Supports other team members in accomplishing their objectives.
Creates ad-hoc reports.
Performs targeted quality control and quality assurance reviews of clients' documentation and workflow Education &
Experience:
Hands-on individual with great attention to detail, high personal accountability, and strong drive to develop him/herself while learning our business model.
Multi-payor experience with provider collaboration preferred Bachelor's degree preferred.
Experience with multiple Electronic Health Records (EHRs) preferred.
Health System experience desired.
Strong organizational skills.
Excellent written and verbal communication skills.
May require travel to practices and trainings out of state.
Infrequent.
Knowledge of HEDIS, Medicare, and ACO practice management preferred.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
